Toronto Public Health is starting the assessment of immunization records for school students as outlined in the OntarioImmunization of School Pupils Act. This year they will be sending letters to students born in 2008, 1999 and 1998.
Information for Parents/Guardian
Please provide updated immunization information to Toronto Public Health. If the student has not been fully immunized, refer them to their family doctor. Free immunization clinics are also available for students who do not have a Health Card or a doctor. Appointments are made at www.tphbookings.ca
Those having difficulty understanding the request for immunization information can call the Immunization Information Line at 416-392-1250. Interpreters are available. The Immunization Information Centre (IIC) offers free translation of immunization records. Photocopy the record including the child’s name, gender and date of birth and send by fax or mail to the IIC for translation. Settlement workers can assist them with this process. Valid Exemption
A small number of parents in Toronto choose not to vaccinate their children. They must sign an affidavit to exempt their child from immunization on the grounds of religion or conscience. Affidavit forms are available by calling the Immunization Information Line at 416-392-1250 and speaking with a nurse. Medical exemptions are completed by a doctor and sent to Toronto Public Health Privacy of Personal Health Information
Under the Personal Health Information and Protection Act (PHIPA), TPH may share documents containing personal health information with school staff under Section 43, 1F. Parents can reasonably expect that TPH will share status of students’ immunization record with school staff. Personal health information must be protected and secured at the school using the following guidelines:
- only school staff who are required to handle student immunization information should have access to students’ personal health information
- keep personal health records in a secure locked cabinet
- when personal health information is no longer required, dispose of it in a secure confidential manner
